Covid-20 – Interim Report

 Even now some of the fundamentals of virology are still not recognized:

  • Coronaviruses travel individually suspended in the air like smoke particles (the same dimensions of 150nm) and like pollens (which seem like their bigger brothers). They are carried by air currents, forced to accumulate in closed environments, so causing harm. The Coronavirus is not a killer virus, but we make it dangerous. In these closed environments they are inhaled and arrive in large quantities at their only point of entry, the peripheral respiratory tract.
  • The necessary viral load for a more serious infection only happens in badly ventilated (through negligence?) environments like care homes, public transport, hospitals, hotels, conference rooms, law courts.
  • The interpretation of the means of transmission of the Coronavirus through droplets and surfaces (as also for the Influenza virus) represents a relic from history, when virology was only a theory, but not a science.
  • The specific death rate from Coronavirus can partly be attributed to the handling of patients with Coronavirus through “autointoxication” (masks, oxygen masks and helmets, one way intubation) and through “external intoxication”: the antiviral drugs.

It is obvious that all this takes the vast majority of government interventions to the point of absurdity, they infringe the most elementary constitutional rights, especially of children.

It is enough to ventilate as required by law (windows open for 10 minutes every hour), and masks, distancing, yellow, orange, red zones, experts and vaccines would become unnecessary.

Introduction: There are currently many different opinions, partly contrasting with regard to the transmission of coronavirus and the measures to be taken to deal with it. At present the main mode of transmission is considered to be through droplets or contact. The aim of this article is to examine the importance of transmission through contact and droplets against transmission by aerosols using an epidemiological approach. To this end three regions of Italy have been compared as cohorts.

Material and Methods: The three regions included in this study are Lombardy, Emilia Romagna and Sicily. A clinical analytical study has been carried out on these three cohorts. The relative homogeneity at the beginning of the epidemic (general risk of exposure, demographic conditions) and eventual interaction during the observation period have been assessed. 24/04/2020 was considered the final date of the survey in relation to fatal cases.

The results were assessed with regard to transmission through contact/droplets or by aerosols.

Results:

Lombardy, Emilia Romagna and Sicily are fairly similar as regards territorial extent (Lombardy: 23,865 km2,, Emilia Romagna 22,453 and Sicily 25,832 km2) As for population/population density  per km2, Emilia Romagna and Sicily are comparable (Emilia Romagna 4,459,477/198.6 per km 2, Sicilia 4,999,891/193.6 per km 2). Lombardy has an approximately double population/population density (10,060,574/421.6 per km2).

The homogeneity of the two cohorts in the north as regards risk of exposure is comparable due to the similar geographical, demographic and climatic conditions. Sicily, 1000 kilometers further south, until the end of March was exposed to a similar if not much higher exposure:

  1. Catania airport (metropolis with over a million inhabitants) took 6th place in Italy last year for passenger transport, in Germany it would be little after Stuttgart, Cologne and Berlin Tegel at 9th place. Compared to the previous year, air traffic in Catania in January increased by 4.94% to 609,750 and in February by 1.36% to 547,210: Lombardy is in third place overall for presences in Sicily (1,069,293) after France (1,584,410) and Germany (1,185,041)
  2. From 3 to 5 February the Festival of St Agatha took place, one of the largest religious festivals in the world with a turnout of over a million people, obviously with no social distancing.
  3. After the closure of universities, bars, restaurants etc. there was a justified return of students and workers from the north: around 40,000 people who registered spontaneously (Note from the Ministry 24/03/20), even if the actual figures will be considerably higher.
  4.  Until International Women’s Day (8 March) as regards the winter season, Catania was the city in Italy and Europe with the highest quota of social personal contacts due to the very intense daily and night life. This was caused by the extensive presence of people both from outside and inside the territory. In this period Catania pulsated from early morning until long after midnight.

As for the homogeneity of the cohorts regarding healthcare and political interference, no differences were found. (The various government decrees applied to the whole of Italy).

26 April 2020 was taken as the final point of the study. According to government statistics the following data were recorded for the number of deaths:

Lombardy: 13,325, Emilia Romagna: 3,386, Sicily: 228)

Discussion: three Italian regions were included in this study, two from the north: Lombardy and Emilia Romagna and Sicily 1000 km further south. The structure of the population is similar with the exception of Lombardy which has double the density of the other two regions. The starting point of the epidemic (end of January, beginning of February) is comparable as regards risk of exposure (possibly higher in Sicily). The lockdown ordered by the government meant that the course of the epidemic was not altered by regional interaction. This allows a homogeneity of the cohorts virtually unique in medicine.

The evaluation of the results regarding the two factors considered responsible for transmission  (droplets/aerosols) leads to the following conclusions:

The importance of transmission through contact, droplets etc. in relation to the course of the epidemic goes towards zero. This is evident from the fact that the necessary interpersonal contact is similar in the three cohorts (certainly much higher in Sicily, especially in big cities), but the death toll in Sicily is very low. (Sanitary conditions, often considered to be the cause of this mode of transmission, are similar in North and South Italy).

The importance of transmission by aerosols remains the only determining factor. The explanation lies in the analysis of the differing living conditions in the home-work environment between the north and the south. Staying in the open air and in insufficiently ventilated environments are identified as determining factors.

The habitat of the Sicilian population, due to the climatic and industrial conditions, is fundamentally different to that in the North.

  1. Buildings are not insulated to guarantee energy efficiency (the concentration of CO2 is an indicator for risk of infection: Rudnick and Milton 2003), they have high ceilings to provide coolness in the summer.
  2. Heating is only used to a limited extent (both in private and public spaces) and there is also sufficient ventilation (see the indications for a healthy climate in buildings) in winter facilitated by the relative difference between the external and internal temperature.
  3. Life in Sicily takes place outside to a great extent also in winter thanks to the mild climate.
  4. Industrial plants with a high concentration of people are far less common than in the North.
  5. There is adequate ventilation in means of transport (cars, buses, trains).

As transmission by aerosols finds an ideal situation in the closed environments in northern Italy (the concentration of CO2 will be caused by the high level of insufficient ventilation) we are faced with, in the broad sense, the already well known Sick building Syndrome (Finnegan et al. 1984). .

The theory of transmission by aerosols is also confirmed in the virological literature. Particles the size of the coronavirus settle at the level of the lower respiratory tract (Bottarelli 2020). The involvement of the oropharyngeal cavity and the upper respiratory tract are to be considered secondary, occurring following exhaled air, not significant for transmission.

Conclusions: As transmission, as explained above, occurs by aerosols (influenza too, showing the same epidemiological features as the coronavirus, probably follows the same mode of transmission) the measures currently being taken (quarantine/lockdown, masks, gloves, measures against gatherings) should be corrected immediately.

It is vitally and urgently important to undertake measures to rectify the human habitat regarding both internal and external pollution.

Covid-20 – ein Zwischenbericht

Bis heute werden einige Grundpfeiler der Virologie nicht anerkannt.

  • Die Coronaviren schweben einzeln frei in der Luft wie die Bestandteile des Rauches (gleiche Dimensionen = 150nm) und wie die Pollen (die ihre groesseren Brueder erscheinen). Sie werden normalerweise vom „Winde verweht“, in geschlossenen Raumen jedoch steigt ihre Konzentration gefaehrlich an. Jetzt eingeatmet kommen sie in hoher Menge an ihre einzige Eintrittspforte, die peripheren Luftwege.
  • Die noetige Viruslast fuer eine ernstere Erkrankung erfolgt also nur in (fahrlaessig?) ungeluefteten Umgebungen (Altersheime, oeffentliche Verkehrsmittel, Krankenhaeuser, Hotels,Justizanstalten etc).
  • Die Deutung der Uebertragung des Coronavirus (und auch des Grippevirus) ueber Droplet, Schmier-und Schmutzinfektion ist nur ein Relikt aus historischen Zeiten, als die Virologie noch eine Theorie und keine Wissenschaft war.
  • Die erhoehte speziefische Coronasterblichkeit ist auf die Versorgung von coronainfizierten Personen zurueckzufuehren (mittels „Eigenvergiftung“ durch Masken, Atemhelme, Einwegintubation und „Fremdvergiftung“ durch antivirale Arzneimittel)

Es ist klar, dass dies alles den Grossteil der schwerstwiegenden Eingriffe in die Grundrechte eines jeden, und vor allem der Kinder, ad Absurdum fuehrt.

Es wuerde reichen, zu lueften wie es gesetzlich vorgeschrieben ist (Stosslueftung fuer 10 Minuten jede Stunde) und Gesichtsmasken, soziale Distanierung, Lockdown, Experten und antiviralle Arzneimittel und Impfungen waeren ueberfluessig.

Einführung: Es liegen zur Zeit viele, zum Teil widersprüchliche wissenschaftliche Meinungen hinsichtlich der Übertragungsweges des Coronavirus und der notwendigen Massnahmen bezüglich der Epidemie vor, wobei die Troepfchen und Schmieruebertragung der Influenza und Coronaviren als moeglicher Hauptuebertragungsweg angesehen wird. Ziele dieses Beitrages ist, ueber epidemiologische Auswertung von Daten in Italien (Kohortenstudie vergleichend Laender von Norditalien wie Lombardei und Emilia Romagna mit Sizilien), den Einfluss der Troepfchen und Schmieruebertragung und der der Aerosoluebertragung gegeneinander zu wichten.

Material: Es wurden drei italienisch Laender ausgewaehlt ( Lombardei, Emilia Romagna und Sizilien). ,

Methode: es wurde eine analytische klinische Studie durchgefuehrt mit den oben aufgefuehrten Kohorten. Deren Homogeneitaet bezueglich Anfangsphase der Epidemie (allgemeines Expositionsrisiko, demographische Verhaeltnisse) wurde evaluiert, eventuelle aussere Einfluesse waerend deren Verlauf, und die Situation zum 26/04/2020 (Endpunkt) bezueglich der Todesfaelle.

Die Ergebnisse wurden hinsichtlich der beiden Uebertragungswege (Uebertragung ueber Troepfchen und Schmierinfektion vs. Uebertragung ueber Aerosol) ausgewertet.

Ergebnisse

Lombardei , Emilia Romagna und Sizilien sind fast identisch bezueglich Ausdehnung. (Lombardei: 23.865 km2, Emilia Romagna 22.453 e Sizilien 25.832 km2.  Bezueglich der Einwohnerzahl/Dichte pro km2 sind Emilia Romagna und Sizilien fast identisch (Emilia Romagna 4459477/198.6 pro km2, Sizilien 4999891/193.6 pro km2). Die Lombardei hat in deren Hinsicht ungefaehr die doppelte Einwohnerzahl/Dichte (10.060.574/421.6 pro km2).

Die Homogeneitaet der Kohorten der beiden Laender Norditaliens bezueglich des Expositionsrisikos ist aufgrund sehr aehnlicher geographischen, demographischen und klimatischen Bedingungen gewaehrleistet. Sizilien, tausend Kilometer suedlicher, war bis Ende Maerz mit Sicherheit einem massiven, vergleichbaren, wenn nicht hoeherem Expositionsrisiko ausgesetzt:

  1. Sizilien belegt mit dem Flughafen von Catania mit ueber 10 Millionen Passagiere im Jahr in Italien den 6. Platz, in Deutschland waere er vor Hannover knapp nach Stuttgart, Koeln und Berlin Tegel auf dem neunten Platz (siehe tab. 1). Weiterhin nahm der Flugverkehr Anfang 2020 in Januar um 4.94% auf 609.750 Passagier und Februar um 1.36 % auf 547.210 zu. Es ist zu beachten, dass die Lombardei (1.069.293 Anwesenheiten), nach Frankreich (1.584.410) und Deutschland (1.185.041) am dritten Platz der Gesamttouristenzahl liegt.
  2. anfang Februar (3-5 Februar) fnd in Catania (einer Metropole mit mehr als einer Million Einwohnern) eines der groessten Heiligenfeste der Welt (Heilige Sant‘Agata) mit einer Beteiligung von mehr als einer Million Besucher statt, ohne auch nur den geringsten Sicherheitsabstand einzuhalten.
  3. nach Schliessung der Universitaeten, Bars, Restaurants etc. erfolgte ein Rueckstrom von sizilianischen Studenten und Arbeitern vom Norden nach Sizilien in der Hoehe von ungefaehr 40.000 freiwillig offiziell registrierten Personen (Mitteilung der Regierung vom 24/03/2020), wobei wohl eine hohe Dunkelziffer besteht.
  4. Bis zum Tag der Frau (8 Maerz) war Catania bezueglich der beiden vorhergegangenen Monate (Januar und Februar) die Stadt Italien/Europas mit der wohl hoechsten Quote der persoenlichen sozialen Kontakten aufgrund des intensiven Tages und Nachtlebens. Dies bedingt durch die grosse Anwesenheit von Personen sowohl von auswaerts als auch vom Territorium. In dieser Zeit pulsierte das Leben in Catania von frueh morgens bis weit nach Mitternacht.

Hinsichtlich bedeutender aeussere Einfluesse wie medizinische Versorgung, politische Entscheidungen wurden keine Unterschiede zwischen den Kohorten festgestellt (die verschiedenen Dekrete der Regierung galten dann ausnahmslos fuer ganz Italien. Dies hat somit einen homogenen Studienverlauf gesichert.

Als Endpunktes der Studie wurde der 26. April 2020 angesetzt. Laut offizieller Mitteilung seitens der Regierung ergaben sich zu diesem Zeitpunkt folgende Zahlen der auf Coronavirus zurueckzufuehrenden Todesfaelle:

Lombardei: 13325, Emilia Romagna: 3386, Sizilien: 228

Diskussion: Diese Studie schliesst drei Laender Italiens ein, zwei Norditaliens (Lombardei und Emilia Romagna und dann Sizilien, tausend Kilometer suedlich. Die Bevoelkerungsstruktur ist vergleichbar mit der Ausnahme, der Lombardei, die die doppelte Bevoelkerungsdichte aufweist. Dier Ausgangspunkt der untersuchten Epidemie (Ende Januar/Anfang Februar) bezueglich des Expositionsrisikos ist vergleichbar, wobei Sizilien eventuell ein hoeheres Risiko aufweist. Der Verlauf der Epidemie wurde dann nicht mehr durch regionale Einfluesse unterschiedlich beeinflusst durch die von der Regierung ausgerufenen Ausgangssperre. Dies alles erlaubt eine in der Geschichte der Medizin (Epidemiologie) wohl einmalige Homogeneitaet der Kohorten.

Die Auswertung der Ergebnisse in Bezug auf die beiden unterschiedlichen Faktoren (Troepfchen , Aerosol) fuehrt zu folgenden Schluessen:

Der Einfluss der Troepfchen- Schmierinfektion auf den Verlauf der Epidemie geht in dieser Studie gegen null. Dies ergibt sich aus der Tatsache, dass der hierzu notwendige persoenlichen zwischenmenschlichen Kontakten in den Kohorten vergleichbar ist, auf Sizilien (vor allem in den Grossstaedten) auf keinen Fall geringer, sondern deutlich ueberdurchschnittlich vertreten ist. Die Todesrate auf Sizilien ist jedoch im Vergleich zum Norden aeussertst niedrig. (Die hygienische Verhaeltnisse, oft als Ursache fuer diese Uebertragung anzusehen, sind zwischen Nord und Sued vergleichbar).

Der Einflusses der Uebertragung mittels Aerosol verbleibt somit als determinanter Faktor. Die Erklaerung hierfuer liegt in der Analyse der unterschiedlichen Lebensgewohnheiten/Umstaende wobei die unterschiedlichen Lebensraeume Nord und Sueditaliens bezueglich der Verweildauer im Freien und in geschlossenen, nicht regelrecht geluefteten Umgebung als ausschlaggebende Faktoren erkannt werden.

Das Habitat der Bevoelkerung auf Sizilien weist aufgrund seiner klimatischen und industriellen Verhaeltnisse, grundlegende Unterschiede zum Norden auf.

  1. Die Innenraeme der Gebaeude sind nicht abgedichtet im Sinne der energetischen Effizienz, haben hohe Decken, um Kuehle im Sommer zu gewaerleisten (die CO2 Konzentration ist mit ein Indikator fuer Infektionsrisiko . Rudnick und Milton 2003)
  2. Heizung wird nur begrenzt eingesetzt (sowohl im privaten als auch im oeffentlichen Bereich) und eine regelrechte Lueftung (siehe Vorgaben fuer Raumklima) wird auch im Winter aufgrund geringer Unterschiede zwischen Aussen und Innentemperatur haeufig durchgefuehrt
  3. Das Leben findet viel mehr im Freien statt, auch im Winter dank des milden Klimas und der laengeren Tageszeit.
  4. Industriebedingte Anlagen mit hoher Angestelltendicht befinden sich vor allem im Norden.
  5. In den Verkehrsmitteln (Auto, Bus, Eisenbahn) wird auf Lueftung geachtet

Da die Uebertragung durch Aerosol geschlossene Umgebung benoetigt, findet sie eine ideale Umgebung im Norden Italiens (die CO2 Konzentration der Gebaeude duerfte aufgrund voellig unzureichender Lueftung im roten Bereich liegen). Somit handelt es im erweitertem Sinn um das seit langem beschriebenem Sick Building Syndrome. (Finnegan et al. 1984)

Hinweise auf die Uebertragung ueber Aerosol finden Bestaetigung auch in der virologischen Literatur. Teilchen in der Groesse des Coronavirus schlagen sich in den tiefen Atemwegen nieder (Bottarelli 2020). Der Befall des Mund-Rachenraumes und der oberen Luftwegen duerfte sekundaerer Natur sein (durch die Ausatmluft bedingt) und traegt nicht zur Verbreitung bei. Dies erklaert auch die aeusserst geringe Infektionsrate der Kinder, die aufgrund ihrer anatomisch-physiologische Besonderheiten des Atemwege die infektioesen Viruspartikel bei ihrem Eindringen in den oberen Luftwegen abfangen und und somit nicht zu ihrer einzigen Eingangspforte, den tieferen Luftwegen gelangen.

Schlussfolgerung: Da der Uebertragungweg des Coronavirus, wie oben ausgefuehrt,  nur ueber Aerosol erfolgt (es ist anzunehmen, dass die Influenzagrippe den gleichen Regeln folgt, da das Infektionsverhalten der letzten Influenzaepidemien denen der Coronaepidemie sehr aehnlich war), gilt es jetzt, die zur Zeit auf verschiedenen Ebenen gesetzten Massnahmen zum korrigieren (Ausgangseinschraenkungen, Gesichtsmaskenpflicht, Versammlungsverbot im oeffentlichen und privatem Bereich wie Kirche, Kinos, Beerdigungen und vieles anderes mehr). Es muss aber sofort aktiv die Sanierung des Lebensraumes der Menschen vorangetrieben werden, um eine gesund Umgebung sowohl in geschlossenen Raeumen als auch im Freien zu gewaerleisten (eine Korrelation von verlaengerter Infektivitaet des Virus im Freien in Gebieten mit erhoehter Luftverschmutzung, wie von Wissenschaftlern behauptet, ist allerdings auszuschliessen).

Covid-20 – Rapporto intermedio

Tutt’oggi non vengono riconosciute alcune delle fondamenta della virologia:

  • i Coronavirus viaggiano sospesi singolarmente nell’aria come le particelle del fumo (stessi dimensione di 150nm) e come i pollini (che sembrano i loro fratelli più grandi). Vengono portati dalle correnti d’aria, costretti di accumularsi in ambienti chiusi, recando allora danno. Il coronavirus non è un virus killer, ma noi lo rendiamo pericoloso. In questi ambienti chiusi vengono ispirati ed arrivano in grandi quantità alla loro unica porta di ingresso, le vie respiratorie periferiche.
  • la carica virale necessaria per una infezione più grave avviene solo in ambienti mal aerieggiati (per negligenza?) come case di riposo, mezzi pubblici, ospedali, alberghi, sale di conferenza, palazzi giudiziari.
  • la interpretazione della via di trasmissione tramite droplet ed superfice del Coronavirus (come anche del Influenzavirus) rappresenta un relitto dei tempi storici, quando la virologia era solo una teoria, ma non una scienza.
  • la mortalità specifica da coronavirus per parte è riconducibile alla gestione dei pazienti affetti da coronavirus tramite ”autointossicazione” (mascherine, maschere e caschi d’ossigeno, intubazione ad una via) e tramite “intossicazione esterna”: i farmaci antivirali.

E’ ovvio, che tutto questo porta ad absurdum la stragrande parte dei interventi dei governi che ledono i più elementari diritti costituzionali, specie dei bambini,

Basta arieggiare come previsto dalla legge (10 minuti aperte le finestre ogni ora), e mascherine, distanziamento, zone gialle, arancioni, rosse, esperti e vaccini diventerebbero inutili.

Introduzione: Attualmente ci sono tanti pareri, per parte contrastanti per quanto riguarda la via di trasmissione del Coronavirus e le misure da intraprendere per affrontarla. Quale via di trasmissione principale viene attualmente considerato quella tramite goccioline o contatto. Scopo di questo articolo è di valutar l’importanza della via di trasmissione mediante contatto e goccioline verso la via di trasmissione tramite aerosol utilizzando un approccio epidemiologico. A tale proposito sono state paragonate quali coorte tre regioni dell’Italia.

Materiale e Metodi: Le tre regioni inclusi in questo studio sono la Lombardia, Emilia Romagna e la Sicilia. E’ stato eseguito uno studio analitico clinico tra queste tre coorte. L’omogeneità relativa l’inizio dell’epidemia (rischio generale di esposizione, condizioni demografiche) ed eventuali interazioni durante il tempo di osservazione sono stati valutati. Quale data di fine osservazione è stata considerata il 24/04/2020 in relazione ai casi letal .

I risultati sono stati valutati per quanto riguarda la trasmissione tramite contatto/goccioline e tramite aerosol.

Risultati:

La Lombardia, l’Emilia Romagna e la Sicilia sono pressoché paragonabile per quanto riguarda l’estensione territoriale (Lombardia: 23.865 km2,, Emilia Romagna 22.453 e la Sicilia 25.832 km2) Per quanto riguarda la popolazione/densità di popolazione pro km2, l’Emilia Romagna e la Sicilia sono sovrapponibili (Emilia Romagna 4.459.477/198.6 pro km 2, Sicilia 4.999.891/193.6 pro km). La Lombardia presenta una popolazione/densità approssimativamente doppia (10.060.574/421.6 pro km2).

L’omogeneità delle due coorte del nord rispetto all’rischio di esposizione sono paragonabile a causa di simili condizioni geografiche, demografiche e climatiche. La Sicilia, 1000 chilometri più al sud, fu esposto fino alla fine di marzo ad una esposizione simile se non molto superiore:

1)L’aeroporto di Catania (metropoli con più di un milione di abitanti) nell’anno scorso per trasporto passeggeri era al 6. Posto in Italia, in Germania sarebbe poco dopo Stoccarda, Colonia e Berlino Tegel al nono posto. Il traffico aereo a Catania rispetto l’anno precedente era aumentato a gennaio del 4.94 % al 609.750 e a febbraio del 1.36% al 547.210: La Lombardia è al terzo posto assoluto di presenze in Sicilia (1.069.293) dopo la Francia (1.584.410) e la Germania (1.185.041)

2) dal 3 al 5 febbraio si era svolta la festa di Sant’Agata, una delle feste religiosi più grandi del mondo con una affluenza di più di un milione di persone, ovviamente senza alcuna distanza di sicurezza.

3) dopo la chiusura delle università, bar, ristoranti etc. avveniva un rientro giustificato di studenti e lavoratori dal nord nella misura di circa 40.000 persone che si sono registrate spontaneamente (nota del ministero del 24/03/2020), anche se i dati reali saranno nettamente superiori.

4) fino al giorno della donna (8 marzo) Catania era per quanto riguarda la stagione invernale la città dell’Italia e di Europa con la quota più alte di contatti personali sociali a causa della vita diurna e notturna molto intenso. Questa era causata dalla massiccia presenza di persone sia di provenienza fuori e entro il territorio. In questo periodo la vita a Catania pulsava dalla mattina presto fino a lungo dopo mezzanotte.

Per quanto riguarda l’omogeneità delle coorte rispetto ad assistenza sanitaria ed interferenze politiche non sono state rilevate differenze. (i vari decreti del governo valevano per tutto l’Italia).

Quale punto finale dello studio è stato considerato la data del 26 aprile2020. Secondo la statistica del governo furono registrati i seguenti dati per quanto riguarda i numeri dei deceduti:

Lombardia: 13.325, Emilia Romagna: 3386, Sicilia: 228)

Discussione: in questo studio sono stati inclusi tre regione italiani, due del nord: la Lombardia e l’Emilia Romagna e la Sicilia 1000 km più al sud. La struttura della popolazione sono paragonabile con l’eccezione della Lombardia che presenta una doppia densità rispetto alle altre due regioni. Il punto iniziale dell’epidemia (fine gennaio, inizio febbraio) è paragonabile per quanto riguarda il rischio di esposizione (nella Sicilia eventualmente superiore). Il decorso dell’epidemia non fu alterato da interazioni regionali a causa della quarantena disposta dal governo. Questo permette una omogeneità tra le coorte pressoché unica nella medicina.

La valutazione dei risultati rispetto ai due fattori ritenuti responsabili per la trasmissione (goccioline/aerosol) permette le seguenti conclusioni:

L’importanza della trasmissione via contatto, goccioline ecc. in relazione al decorso dell’epidemia va verso zero. Questo si evince dal fatto che i contatti personali interumani, necessari per questo, è paragonabile nelle tre coorte (in Sicilia, specialmente nelle grandi città sicuramente molto superiore), ma la mortalità in Sicilia risulta molto bassa. (le condizioni igieniche, spesso considerato causa per questa via di trasmissione, sono paragonabile fra Nord e Suditalia).

L’importanza della trasmissione tramite aerosol rimane l’unico fattore determinante. La spiegazione per questo sta nell’analisi delle diverse condizioni di vita tra l’ambiente abitativo – lavorativo tra nord e sud. La permanenza all’aria aperta ed in ambienti non sufficientemente aerati vengono individuati quali fattori determinanti.

L’abitat della popolazione siciliana presenta, a causa delle sue condizioni climatiche ed industriali delle differenze fondamentali rispetto a quello del nord.

  1. Gli edifici non sono isolati per garantire una efficienza energetica (la concentrazione di CO2 costituisce un indicatore per il rischio di infezione: Rudnick e Milton 2003), presentano dei volti alti, per permettere freschezza d’estate.
  2. Il riscaldamento viene solo in misura ridotta utilizzato (sia in ambienti privati che pubblici) e una aerazione sufficiente (vedi indicazioni per un clima sano nei edifici) avviene anche d’inverno favorito dalla relativa differenza tra temperatura esterna ed interna.
  3. La vita in Sicilia si svolge all’esterno anche in misura intensa d’inverno anche grazie al clima mite.
  4. Impianti industriali ad alta concentrazione di persone sono molto meno diffusi rispetto al nord
  5. Nei mezzi di trasporto (automobili, bus, treni) viene rispettata un’aerazione adeguata

Siccome la trasmissione attraverso aerosol trova una situazione ideale in ambienti chiusi nel nord dell’Italia (la concentrazione del CO2 sarà a causa della insufficiente aerazione a livelli elevata) ci troviamo, nel senso largo, d’aventi algià da tempo noto Sick building Syndrome (sindrome della casa ammalata) (Finnegan et al. 1984).

La teoria della trasmissione tramite aerosol trova conferma anche nella letteratura virologica. Particelle della grandezza del coronavirus si depositano a livello delle vie aerea basse (Bottarelli 2020). Il coinvolgimento del cavo oro-faringeo e delle vie aeree superiori sono da considerare secondarie, avvenute in seguito dell’aria espirata , non determinante per la trasmissione.

Conclusioni: Siccome la via di trasmissione, come esposto sopra, avviene attraverso l’aerosol (anche l’influenza, mostrando le stesse caratteristiche epidemiologiche del coronavirus, segue probabilmente la stessa via di trasmissione) vanno corrette immediatamente le misure attualmente intraprese (quarantena, maschere, guanti, misure antiassembramento)

E’ di fondamentale ed urgentissima importanza di intraprendere delle misure per sanare l’abitat dell’uomo sia per quanto l’inquinamento interno che esterno
